What to Expect on Your First Visit
Your initial visit will include a comprehensive evaluation by our licensed chiropractor. During this appointment, we will:
Review your medical history and current symptoms
Perform a physical assessment to evaluate movement, strength, balance, and flexibility
Discuss your personal goals for therapy
Develop a customized treatment plan tailored to your needs
Your first session will typically last 45 to 60 minutes, and we recommend wearing comfortable, loose-fitting clothing that allows for movement.

To save time at check-in, please complete the following forms before your first visit:

Medical History Questionnaire
HIPAA Privacy Policy Acknowledgment
Forms can be completed online or downloaded from our website and brought to your appointment. If you prefer to fill them out in person, please arrive 15 minutes early to allow time for paperwork.
What to Bring
For your first appointment, please bring:
A valid photo ID
Any Medical Records related to your condition
A list of current medications
Appointment Policies
Cancellations & Rescheduling: Please notify us at least 24 hours in advance if you need to cancel or reschedule your appointment.
Late Arrivals: If you are more than 15 minutes late, we may need to reschedule your appointment.
What to Wear: Comfortable clothing and supportive footwear (such as sneakers) are recommended.
